Hello friends! We are looking forward to welcoming you to Bar Lazy J! Please take a moment to complete the following information for your party so we can get things in order for you here. This information is required for EACH individual/couple/or family joining us. We're excited to see you soon!

If you will be arriving by Home James or GoAlpine, please let us know what time your shuttle drop-off is so that we can be there to greet you. If you would like us to make a stop in town for anything before we head to the ranch, just ask! We're happy to do so.

If you are driving or arriving by private shuttle, what time shall we anticipate your arrival? Remember check-in is 3pm.

Does anyone in your party have any dietary or other allergies we should be aware of? (food, animals, plants,etc)? If so, provide details.

Does anyone have any dietary restrictions they intend to adhere to on vacation?

Our chef's work very hard to accommodate allergies and special dietary needs of our guests. They are happy to prepare special dishes to accommodate your needs if you are planning on following your dietary restrictions while on vacation.

Does anyone in your party have any physical limitations or health concerns that we should be aware of (pregancy, medications such as blood thinners, recent procedures, history of respiratory/heart problems, phobias, etc.)?

This information will be kept confidential, but due to current health concerns, this section is very important and will allow us to best help you in case of an emergency.

Please list the NAME OF EACH PERSON in your immediate party, AGES at arrival, HEIGHT and WEIGHT, approx. DATE OF LAST RIDE and each guests' LEVEL OF HORSE EXPERIENCE on a scale from 1-5: 1-never ridden or has ridden a few times; 2- rides a few times a year; 3-rides weekly; 4-rides daily, 5-trains horses (Ex. John Smith/14/5'4/120lbs/3-25-19/Level 3) Please include ALL information for EACH person *

This information is needed in order to assign you a horse for the week. If you are not intending to ride, please just include name and write "non-rider".

For returning guests, let us know if there is a specific horse you'd like to request back. Although various factors don't allow us to guarantee your request, we'll do our best to accommodate them when we can.
